New Patients

We kindly request that new patients fulfill two requirements:

1. Please fill out this New Patient Form prior to your visit.
2. Call our office at 206-623-8405 to schedule your appointment.

We are located on the 16^th floor of the Medical Dental Building in downtown Seattle, Washington.

The most convenient parking is available at Pacific Place Mall which is just across the street from our office, between 6th and 7th Avenue. There is also valet parking available in the Medical Dental Building.
